Welcome to the The Forum Health Centre website, where you can access the wide range of health services available at our surgery.
The Forum Health Centre is a well established group practice in North East Coventry. We look after 14,000+ patients, with 7 doctors and 4 nurses. We are a GMS Practice (General Medical Services), teaching medical strudents (Warwick University) and training GPs.
Our doctors have a diversity of medical interests and skills - we have a commitment to the highest standards, and we hope that our patients make considerate use of our NHS facilities as we aim for continuous improvement as resources permit.
We are committed to giving the best possible service to our patients and so in summary we ask that you:
- Arrange appointments for as far in advance as is appropriate.
- Please help other patients by not requesting an urgent appointment for a problem that can wait a day or so (Mondays are especially busy).
- If you cannot keep your appointment please let us know as soon as possible as it can always be used by someone else.
- Please report to reception on arrival for your appointment.
