Forum Health Centre are proud to be based in world-class facilities following a £2.4m rebuild completed in July 2016. Facilities at the Practice are include a large waiting room, GP and Nurse consulting rooms, treatment room, access for wheelchair users, toilet and baby changing facilities and patient education and meeting facilities.
Parking is available to all patients, including disabled parking to the front of the surgery. Please do not block the entrance of the surgery as this is in constant use by the Doctors and Ambulance Services.
We are working closely with UHCW hospital and the Coventry and Warwickshire Partnership Trust to provide services once provided in the hospital to our new surgery including diabetes care, sexual health services, mental health services and carers support.
We currently have nine GPs - six Partners, three salaried GPs and three trainee GP registrars. Three Nurses who run various clinics and are able to help and advise with problems that do not require a doctors attention including chronic health conditions, diabetes, asthma and respiratory conditions and minor illness. One Clinical Assistant responsible for blood tests (Phlebotomy), NHS health checks and one part-time Phlebotomist.

Vocational Training for General Practitioners
The Forum Health Centre participates in the advanced training of doctors who wish to become GPs. GP registrars are fully qualified doctors who have worked in hospital posts for two or more years, please accept their services as an extension of our own.
In some consultations Video cameras are used in training sessions. You will be informed prior to booking this appointment if your consultation is being recorded and only if you have consented.
In addition, we sometimes have medical students working with us for a short period. We hope you will co-operate with us in giving students this valuable experience. However you do have the right not to have a medical student present whenever you see your doctor.
Mrs Bernadette Grimshaw is responsible for the day to day running of the practice and administration staff. She is happy to help you with non-medical problems and welcomes your views and suggestions provided by the practice.
Our friendly, helpful and team of receptionists are always happy to help and assist you in making appointments, prescription ordering, dealing with any questions or enquires, and happy to offer help and assistance at all times.
Secreterarial and Administration Staff
Our efficient team of medical secretaries and administration staff deal with referrals, registrations, private ‘non-nhs’ claim reports, scanning of medical letters and reports, and other work generated within the practice.
Our prescription staff are happy to assist you with your prescription orders and queries. Please be advised that repeat prescriptions will be available 48 working hours after being requested, and does not include the day it is requested. Please note, we are unable to take prescription requests over the telephone, unless prior authorisation has been given.
