The majority of procedures, consultations and services carried out at this surgery are paid for by the NHS. However, there are some services, such as certain vaccinations and completion of medical reports and forms, which are chargeable as they do not fall under the terms of service of the NHS Standard Contract.
Our priority is NHS work. Non-NHS work is charged in line with British Medical Association Guidelines.
Most frequently requested non NHS services include:
- Certificates, Forms and Letters:
- Private Insurance forms
- HGV license forms
- Holiday cancellation forms & medical reports
- Fitness to travel certificates
- To whom it may concern letters
- Passport Forms*
- Private prescriptions
- Vaccination services - includes Yellow Fever and Hep B injection.
*The Doctor is only allowed to sign a passport application if they have known you personally for a minimum of two years.
Please note that GPs are not obliged to do any of the above work, it will always take second place to their clinical work. A £5 deposit is now required when requesting any release of domentation or information. The desposit will the be deducted from the total fee and outstanding payment is due prior to any documentation release. Our fees for these services are displayed in the surgery, however you will be notified of the charge prior to completion if requested. Private work can take up to 28 days to complete, this is in line with BMA approved national guidelines.
Accessing Medical Records
The practice is registered and complies with the Data Protection Act 1998 and General Data Protection Regulation (GDPR) 2018. Any request for access to notes by a patient, patient’s representative or outside body will be dealt with in accordance with the Act. Requests for information should be submitted in writing with the patient's signed authorisation included for information to be released. Direct requests by patients will be asked to complete a 'medical consent' form given by reception. If you require further information, please speak to a member of our reception team.
