Care Coordination at Forum Health Centre

Our reception and admin staff have received training to be able to support you with your queries and book or signpost you to the most appropriate clinician or service. When calling the practice, our team may ask for a brief outline of your concern. This information will be treated with the strictest confidence and all of our staff are required to follow our confidentiality policy.

The practice has many different clinicians and services we are able to book and refer you to. Signposting you to the most appropriate service helps you to get the right care first time.
