Social Prescribing Service
The practice is supported by a team of social prescribers who work across the ten GP Practices in the Sowe Valley Primary Care Network to help patients with a range of non-medical needs.
What is social prescribing?
Social prescribing, also sometimes known as community referral, is a means of enabling health professionals to refer people to a range of local, non-clinical services.
Social prescribing is designed to support people with a wide range of social, emotional or practical needs, and many schemes are focused on improving mental health and physical wellbeing. Those who could benefit from social prescribing schemes include people with mild or long-term mental health problems, people with complex needs, people who are socially isolated and those with multiple long-term conditions who frequently attend either primary or secondary health care.

Beth Darling-Bates is the main social prescriber who provides support to our patients.

How can I get in touch with the social prescribing team?
If you would like to be referred to the social prescribing team for support, speak to a member of the practice team. The surgery will refer you directly and someone from the team will contact you within 14 days.
