Use our Patient Triage Form for non-urgent appointments and queries. If you have multiple queries, please complete a form for each one.

Our online consultation service is open to requests 7 days a week. Your non-urgent request will be read and reviewed within core hours (8am-6:30pm Monday to Friday). Requests submitted during our core hours will be responded to by the end of the working day. Requests received outside of core hours will be responded to by the end of the next working day.
