LONG TERM CONDITION REVIEWS

Patients with a long term condition require annual review of their health and medication to ensure future health risks are minimised and that you are receiving the most appropriate treatment.

What conditions require annual monitoring?

As a general rule any condition that requires you to take regular medication will require annual monitoring. Conditions include:

  • Asthma and COPD (breathing conditions)
  • Coronary Heart Disease (atrial fibrillation, heart attacks and heart failure)
  • Diabetes
  • High blood pressure (hypertension)
  • Hypothyroidism (under-active thyroid)
  • Stroke (CVA)

Why have a long term condition review?

Patients are expected to attend a minimum of once each year for review of their condition and medication and you will be sent a reminder SMS message when you are due.

It is important that you respond promptly to your invitation as this allows us the opportunity to ensure your condition is well controlled, that you are prescribed the most appropriate medication and that there are no side effects from your medication.

Most conditions apart from asthma and COPD will require a blood test which allows us to check the function of important organs and cardiovascular risk factors.

Please attend promptly for review when invited. Patients who fail to respond to repeated invited will have their medication quantities reduced to weeklu for safety reasons and may be considered for removal from the practice list if we feel we are no longer able to safely care for you.
