At Forum Health Centre, we understand the increasing demands on modern life and the differing needs of our patients.
We want to be able to give you flexible access to the best medical care when you need it. This is why from May 2018 we are able to offer Video Consultations over our online patient system to allow patients to attend appointments at home or even while at work, giving them more options to recieve care.
How does a Video Consultation work?
Video consultations are very similar to Telephone Consultations, however there is a camera allowing you to see your GP or Nurse and the GP or Nurse to see you. When you click to enter your Video Consultation, it will be a secure connection that only yourself and the GP or Nurse can access.
How can I book a Video Consultation?
Video consultations are availble to be booked online via our Online Appointments Portal, Patient Access.
If you do not have Online Access to book appointments, please complete this form and bring it into the Practice for access to be granted. (Please be aware that you will need to bring 2 forms of ID, 1 photographic and 1 proof of address, for Data Protection Purposes).
If you already have Online Access to book appointments, but do not have a Patient Access account, please click here to set one up.
Please be aware that all Video Consultations must be booked online.
When you book your appointment for a Video Consultation, you are confirming that:
You have access to a computer or laptop with a webcam, or a smartphone with a front-facing camera.
You understand that it is your responsiblity that you are in a safe, confidential environment to discuss your health.
If you have booked a Video Consultation, and you no longer feel like you are in a secure public place to discuss your health, however feel a telephone appointment would be more beneficial. Please contact our Reception Team as soon as possible and we will be able to ensure the GP or Nurse call you via telephone instead ot re-arrange your appointment.
How do I start my Video Consultation?
